Palliative Home Care is a specialized team of caregivers whose support enables you to remain at home. This team consists of the:
- Palliative Home Care Nurses
- Palliative Home Care Aides
- Other members of the palliative care team as described and included in this website (i.e. social work, chaplain etc.)
The Palliative Home Care Nurse visits you in your home and provides:
- management of pain and symptoms
- support for you and your family
- on-going care needs assessment
- liasion between you, your doctor and the Palliative Care Coordinator
The Palliative Home Health Aides provide support through various home services which include:
- personal care
- respite care providers
- assistance with homemaking and meal preparation
Palliative Home Care Nurses are available on a 24-hour basis. Home Health Aides provide services at various times in a 24-hour period based on client needs (as assessed by your Palliative Care Coordinator).
The Palliative Home Care Program encourages family involvement in planning and providing care to the extent that you desire. The interdisciplinary team identifies and honours you and your family's preferences through careful attention to your values, goals and priorities, as well as your cultural and spiritual background.
We appreciate the many physical, emotional and financial demands of being cared for at home. The program will provide support services to your family such as respite, 24-hour support by telephone, personal care assistance and referral to community agencies and resources.
You, your family, a friend or a health care professional can call 306-766-7200 to request palliative services. A referral will then be made to the Palliative Care Coordinator, who will arrange to visit you. The coordinator, you and your family will discuss your care needs, and develop a coordinated care plan with you.
