The Palliative Care Medical Social Worker may assist you and your family cope with the challenges that can result from illness, hospitilization and disability.  The Medical Social Worker provides assistance with counselling especially in relation to dealing with the psychosocial and emotional needs of clients and their families.  They can assist you with questions pertaining to illness adjustment, health benefits, legal concerns, home care services, funeral arrangements, family counseling and other matters of personal concern.  The Medical Social Worker meets with clients and families both in hospital and in the community.

 

The Social Worker helps guide the emotional and social needs of your illness.  Support and information could include:

 

  • planning for the future
  • preparation of your will/estate
  • settling of financial issues
  • appropriate referrals to community resources
  • discharge and care planning
  • bridging communications within the family
  • examining emotional issues
  • assistance with losses, such as:

- material losses, income, surroundings

- relationship losses, role loss, health and function losses

Your Palliative Care Coordinator, you, a family member, or your significant other may call the Social Work Office at 306-766-2597 or 306-766-2501.  You may leave a confidential message on the voice mail and will receive a reply as quickly as possible.

 

A time can be arranged for you and your family (including your children) to meet with the Social Worker wherever you prefer-be it at home, in the hospital or a long term care facility.
