Palliative Care Services

The Palliative Care coordinators will meet with you to discuss and plan for Palliative Care Services.  Your coordinator is responsible for and will assist you and your family with assessment of your needs, care coordination, planning and implementation of your care.

 

The Palliative Care coordinator is responsible to assure that changing needs and goals are met and to facilitate communication and continuity of your care.  The coordinator will strive to meet your preferences about care settings and promote a seamless transition between home, hospital or other setting of care as needed.

 

You, your family, a friend or a health care professional can call 306-766-7200 to request services.  A referral will then be made to the Palliative Care coordinators who will arrange to visit you.  The coordinator, you and your family will discuss your needs, developing a coordinated care plan.
